Advanced Nurse Practitioner – Care Homes
Are you after a new challenge? Wythenshawe Primary Care Network (PCN) is looking for a clinical team to provide enhanced care to residents in nursing and residential care homes linked to the practices in the area.
The advanced nurse practitioner will take an active leadership role in the evolution of the team and will be the lead for the service and provide assurance to the Board (representing the seven practices in the PCN).
The team will be involved with the day to day care of approximately 350 patients, including elderly and learning disability residents. They will comprise initially general practitioner, advanced nurse practitioners, clinical pharmacist. Other roles will be added as the team develops and systems and processes are embedded.

The post holder will be an autonomous qualified practitioner who will be expected to take a lead in the delivery of the Enhanced Health in Care Homes requirements in residential and nursing homes (care homes) in the Wythenshawe (PCN area).

You will be providing comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnostics, treatment planning, decision making, non-medical prescribing and evaluation.

Working a multidisciplinary team in primary care in South Manchester, you will be responsible for co-ordinating the provision of specialist interventions to meet the health care needs of people in their place of residence.

The post holder will provide specialised input as an independent and interdependent practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in a variety of health care settings in Wythenshawe.

As a senior clinical decision maker, you will aim to prevent unnecessary acute hospital admissions working in the best interests of complex and vulnerable care home residents.
You will assist patients and carers to proactively manage and anticipate their future health needs and develop appropriate plans, in conjunction with their GP and other medical staff.
