Are you looking for a new challenge? Wythenshawe Primary Care Network is looking for a clinical team to provide enhanced care to residents in nursing and residential care homes linked to the 7 practices in the area.
The Clinical Pharmacist will support care home and practice staff with prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, proving expertise in clinical medicines advice while addressing both public and social care needs of patients.
This is an exciting opportunity to join the brand new team that will be providing high quality care to residents and you will be key in the establishment of how the team will work and also the setting up and delivery of the service by the team. You will be involved from the outset and have an input into how the team works going forward.
The team will be involved with the day to day care of approximately 350 patients, including elderly and learning disability residents. The team will comprise initially of general practitioners, advanced nurse practitioners, clinical pharmacist and nursing associate. Other roles will be added as the team develops and systems and processes are embedded.
The post holder will work directly with multidisciplinary teams to improve clinical effectiveness and person-centred outcomes. The role will be based within the GP member practices/Care homes providing support to the Care Homes within the PCN, GPs and other healthcare professionals within the teams to improve the health of patients through the rational and safe use of medicines. The post holder will contribute to the efficient working of the PCN Care home team by leading on key areas in relation to medicines optimisation in Care Homes. The role will include face to face and remote patient contact.
