Practice Nurse, Dr M K Patel and Partners, Wigan

Dr M K Patel and Partners are looking for a Practice Nurse to undertake a wide range of nursing duties, including; being responsible for the effective management of patients with long-term conditions, health promotion, screening and immunisations as well as actively supporting the practice management team in the reviewing and delivery of clinical policies and procedures.

As a practice nurse you will demonstrate critical thinking in the clinical decision-making process. You will work collaboratively with the general practice team and wider Primary Care Network to meet the needs of patients, supporting the delivery of policy and procedures.

Dr M K Patel & Partners will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks.
• Deliver and promote screening programmes
• Carry out ECGs and observations as required
• Ordering and stock control of medical consumables
• Emergency drug stock monthly checking
• Delivery of travel clinics
• Infection prevention control
• Implement and embed health promotion
• Evaluate individual treatment plans for chronic/long term disease patients
• Identify and prevent patients at risk of developing long-term conditions
• Review patient medications linked to their conditions
• Process pathology results linked to long-term conditions
• Maintain accurate clinical records and ensure SNOMED terms are used effectively
• Support the full team in dealing with clinical emergencies
• Recognise, assess, and refer vulnerable patients in line with local pathways
• Liaise with external services/agencies to ensure the patient is supported
• Promote and deliver vaccination programmes for adults and children
• Support the clinical team with all safeguarding matters
• Actively take part in shared learning, significant event analysis and clinical audits and quality improvement
• Develop, review and implement nursing protocols
• Participate in local initiatives/enhanced services
• Review and respond to local and national policies and changes

PERSONAL SPECIFICATION
Essential
Registered Nurse (Nursing and Midwifery Council)
Experience of working autonomously
Experience of infection prevention and control measures
ECGs
Processing pathology results, advising patients accordingly
Understand the requirement for PGDs and associated policy
Ability to record accurate clinical notes
Ability to work within own scope of practice and understanding when to refer to GPs
Excellent communication skills (written and oral)
Effective time management (planning and organising)
Ability to work as a team member and autonomously
Good interpersonal skills
Problem solving and analytical skills
Ability to follow clinical policy and procedure
Strong IT skills
Polite and confident
Flexible and co-operative

Mentor or teaching experience
Post graduate diploma or degree (Chronic Disease Management)
Experience of working in a primary care environment
Experience of chronic disease management
Experience of working as a practice nurse or community nurse
Experience of managing elderly and housebound patients
Women’s health (cervical cytology, contraception, etc.)
Immunisations (routine, childhood and travel)
Chronic disease management
Travel medicine
Knowledge of health promotion strategies
