Nursing Associate

In Wythenshawe Primary Care Network (PCN) we are passionate about improving the health and wellbeing of all our Care home patients.

Are you looking for a new challenge? Wythenshawe PCN is looking for a clinical team to provide enhanced care to residents in nursing and residential care homes linked to the practices in the area.
The post holder will be involved with the day to day care of approximately 350 patients, including elderly and learning disability residents. The team will comprise of initially GPs, Advanced Nurse Practitioners, Nursing Associate, Clinical Pharmacist and an administrator. Other roles will be added as the team develops and systems and processes are embedded.

Experience of Care home nursing would be an advantage, but is not essential.

You will work as a member of our Nursing team, under the guidance of an Advanced Nurse Practitioner, support the organisation and facilitation of clinical care in a holistic and proactive manner to the approx. 350 patients registered in our Care homes for elderly and learning disability patients.

The successful candidate will have completed the Nursing Associate foundation degree and be registered with the NMC and there will be the opportunity to take your skills to the next level
