Community Paramedic, Conway Road Medical Practice, Trafford

Job summary
We are looking for an enthusiastic and motivated Community Paramedic to join our Practice team. We are a high achieving and very well regarded practice in Sale, Trafford. This is a full time or part time post but the hours of work can be flexible.
The candidate should be able to demonstrate safe clinical decision making and expert care of patients within our Practice. They should be competent in history taking and clinical assessment, diagnosis and treatment.
The successful candidate will be working alongside a multi-disciplinary general practice team to meet the needs of the patient. The Paramedic should be able to triage patients effectively and manage clinical cases (mostly minor illnesses) within a primary care setting.
Main duties of the job
The main responsibilities of the role include:
Working closely with our reception team to coordinate appropriate patient triage/signposting.
Work as an autonomous, accountable Paramedic, providing a holistic approach including assessment, diagnosis, and treatment to deliver high-quality patient services in the home, nursing home or practice.
Work closely together, and as part of a wider multi-disciplinary workforce including, GPs, pharmacists, nurses, social prescribers and first contact practitioners.
Use advanced clinical skills to provide education to patients and service users, promoting self-care and empower them to make informed choices about their treatment.
Assess, diagnose, treat, refer, or signpost patients with undifferentiated or undiagnosed conditions relating to minor illness or injury, supported by senior clinicians as needed.
Working with our care home lead to provide safe and high quality care to our care home patients.
About us
We are a friendly and supportive GP Practice located in Sale, Trafford, with 13000 patients. We are a team of 10 GPs, 5 practice nurses, pharmacists and First Contact Practitioners.

Essential
• a Qualified paramedic with at least 2 years experience.
