Job summary

We have an exciting opportunity for an Advanced Nurse Practitioner (ANP) to join our growing General Practice Team. We are a patient centered forward thinking practice based in Central Bury, with a mixed and diverse patient based. We provide care to just over 10k patient.

This role is an integral part of the exciting and innovative transformation of Primary Care, working within our locality. The post holder will work collaboratively with the multi-disciplinary General Practice team and particularly with a dedicated Duty Doctor as part of our Acute Care Team, whilst maintaining and developing their Chronic Disease management skills.

You will be an autonomous practitioner working independently and in conjunction with other health care professionals. You will assess, diagnose and treat patients with a range of minor illness and injuries, acute and chronic medical conditions and initiate direct referrals as appropriate. This is a new role, therfore we are seeking an experienced ANP to act as an autonomously.

Main duties of the job

The post holder will be competent in general practice using their expert clinical knowledge and skills. They will have the authority to make autonomous decisions in the assessment, diagnosis and treatment of patients.

The post holder must be able to independently prescribe appropriate medication, evaluate or refer to other specialists if necessary andensure that patients fully understand any procedure to be carried out on them, and that the appropriate consent is obtained and documented in the Emis Web clinical system.

Include (but not limited to);

1. To manage and develop a clinical acute and chronic case load, dealing with presenting patients needs in a primary care setting

2. Prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice and national and practice protocols.

3. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

4. To deliver a high standard of patient care using clinical specialist practice skills

About us

We are a friendly, modern practice with a combined patient population of just over 10,000 patients.

We have an excellent clinical team including 6 partners, 2 Practice nurses, 1 HCA and Medical students.

We also work very closely with the Additional Roles Staff in our PCN team. Included in this is the AVS (Acute Visiting Service), Pharmacists, First contact Physio therapists and EWH (Extended Working Hours)

We are a training and teaching practice providing support to ST2 currently.

We are keen to train and develop our own practice staff and encourage personal development at every opportunity.

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Work autonomously and be accountable for his/ her professional actions.
  • To undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to registered patients.
  • Assess patients with a range of acute, non-acute, and chronic medical conditions. To take history, conduct appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow-up.
  • Where appropriate make referrals to other healthcare professionals in primary and secondary care.
  • Arrange hospital admission for appropriate cases.
  • Undertake telephone triage work and provide telephone advice calls as appropriate.
  • Prescribe medications as appropriate.
  • Maintain accurate record keeping in Emis Web.
  • Undertake regular medication reviews and medicines management.

Person Specification

Qualifications

Essential

  • Essential
  • Registered Nurse with Nursing and Midwifery Council
  • Masters degree required for qualification post December 2020
  • (See RCN Credentialing for Advanced Level of Nursing Practice)
  • Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Meets the standards for registered ANP working at advanced level

Desirable

  • Qualified triage nurse

Experience
