A rare opportunity has arisen within the practice for an enthusiastic and motivated Advanced Nurse Practitioner (prescriber) to work at The Cheetham Hill Medical Centre, joining a well-established successful nursing team of 2 other ANPs 1 Nurse Consultant/Manager, and 2 very experienced Practice Nurses, as well as 3 healthcare assistants who also compliment the self-contained nursing team who work in conjunction with a great team of Doctors/Pharmacists/PAs and a very supportive management and administrative function.
The practice is recognised as a forward thinking organisation who were originally the first practice in the area to employ an ANP; as well as the first practice to adopt a joined up, collaborative approach to working with other local practices forming a group of three contracts, currently servicing in the region of 22,500 patients across the group.
There are three sites across the group however you will almost exclusively be working from the Cheetham Hill Primary Care Centre; where the building is also host to a plethora of other community services such as the District Nurses, Midwifery Services, Health Visitors as well as a Pharmacy on site.
To act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care.
Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
Undertake Clinical Nursing Practice at an advanced level using expert knowledge and clinical skills to deliver holistic care for the registered patients.
Ensure clinical standards meet all Care Quality Commission and Service Level Agreement Standards of care to people accessing primary health care services.
Assess, diagnose and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
Assess, diagnose and treat patients who require acute medical attention and refer to appropriate agency
Directly admit patients to secondary care hospital in acute medical need.
Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray, bloods and microbiology.
Work within professional guidelines and codes of conduct.
Although the post is for a person to work full time, we are also open to adjusting this to multiple part time positions as the practice is always seeking to invest in the right individuals for our patients.
